The Local Government Association, NHS Confederation, NHS Clinical Commissioners and NHS Providers have come together to help spread the learning from the vanguard programme more widely across the health and care sector.
The dissemination programme has enabled vanguards to share the learning from the first year through a wide range of products from podcasts, an animation, factsheets, regional events and visits. The first event took place in Leeds in March, and this second meeting in London is being chaired by Chris Hopson, chief executive of NHS Providers.
The event, ‘New models of care and the prevention agenda: an integral partnership’, will showcase a examples of great care from around the country, demonstrating how improved preventative care can rebalance the relationship between the individual and the system.

The day is aimed at enabling services to share learning on the implementation of new care models and will look specifically at how to address the health and well-being gap outlined in the Five year forward view, through the development of new care models and by collaborating with local partners.
As well as Professor Sir Muir Gray, delegates will hear from Samantha Jones, director of the new care models programme at NHS England. Vanguard sites Sutton Homes of Care and West Wakefield Health & Wellbeing Ltd will present case studies.
